Treatment Plans

Doctor's orders and treatment directives visible to nursing staff for coordinated patient care.

Overview

Treatment Plans are doctor’s orders that provide nursing staff with clear directives for patient care. They bridge the gap between the doctor’s consultation and the nurse’s daily care routine.

Creating a Treatment Plan

During or after a consultation, the doctor can create/update the treatment plan:

  1. Open the patient’s consultation
  2. Scroll to Treatment Plan & Orders section
  3. Enter:
    • Diagnosis — current working diagnosis
    • Treatment Plan — care instructions, interventions, monitoring requirements
    • Medications — current medication orders (separate from pharmacy prescriptions — these are nursing-administered)
    • Special Instructions — alerts, restrictions, escalation criteria
  4. Click Save Treatment Plan
Treatment plans are per-patient (not per-visit). Each save updates the same plan with a new version. The full history is preserved.

Where Treatment Plans Appear

Treatment Plan History

Every update is versioned. The nursing chart shows:

  • Current active plan (most recent version)
  • Who last updated it and when
  • Expandable history showing all previous versions

This ensures continuity of care across shifts — incoming nurses can see exactly what the doctor ordered and when it was last reviewed.

Permissions

  • Create/Update: Requires hc-consultations permission (doctors)
  • View: Requires hc-nursing-care or hc-ward-management or hc-view-patients
